By completing and submitting the form below you are accepting the following:
Waiver of Liability
In consideration for being permitted to participate in the Epiphany swim event, organized by Serbian Orthodox Church Saint Sava, Vancovuer, I, the undersigned, acknowledge and agree to the following terms:
Assumption of Risks: I understand that participating in an ocean swim involves inherent risks, including but not limited to unpredictable weather conditions, strong currents, marine life encounters, and the potential for injury. I voluntarily assume all risks associated with my participation.
Physical Fitness: I hereby certify that I am physically fit and have sufficiently trained for the ocean swim. I am not aware of any medical condition that would prevent my safe participation in the event.
Release of Liability: I release and discharge Serbian Orthodox Church Saint Sava, Vancovuer, its organizers, sponsors, volunteers, and any other entities associated with the event from any and all liability for any injury, loss, or damage to person or property that may arise from my participation in the ocean swim.
Medical Treatment Authorization: In the event of an injury or illness, I authorize event organizers to seek medical treatment on my behalf and I assume all associated costs.
Photography Release: I grant permission for the use of my name and image in event photographs, videos, and marketing materials.
